Documents for a Slovakia visa application
The papers third-country nationals typically assemble for a Slovak national (Type D) visa and a temporary-residence permit application.
Important
Slovakia's national long-stay route has two document layers. The national (Type D) visa itself needs a short core set, but because a Type D visa is almost always issued to enable a residence permit, the substantive documentation is the residence-permit file assembled for one specific purpose (business, employment, study, research, family reunification or an EU Blue Card). A Slovak temporary residence permit is, by law, bound to a single purpose, so the supporting documents differ by that purpose.
All foreign public documents must be apostilled or superlegalised, and any document not in Slovak or Czech must carry an officially certified translation into Slovak. Documents proving accommodation and criminal-record extracts have limited validity windows, so timing matters.
Core national (Type D) visa documents
- Completed and signed 'Application for National Visa' form
- A valid travel document (passport) valid at least three months beyond the intended validity of the visa
- Recent passport photographs to the required specification
- Proof of the purpose that the visa serves (most commonly the granted residence permit or the residence application)
Typical temporary-residence supporting documents
- Official temporary-residence application form issued by the Ministry of Interior
- Documents proving the specific purpose (e.g. employment contract/work confirmation, letter of admission to studies, business/company documents, or family-relationship certificates)
- Criminal-record extract from your country of origin and from every country where you stayed more than 90 days within six consecutive months in the last three years (apostilled or superlegalised)
- Proof of accommodation in Slovakia, not older than 90 days on the day of submission
- Proof of sufficient financial means for the stay
- Two recent photographs
Certification and translation
- Foreign documents must be apostilled or superlegalised depending on the issuing country
- Documents not in Slovak or Czech require an official certified translation into Slovak
- After the card is granted, a medical report and Slovak health-insurance confirmation are submitted separately (see 'After you arrive')
