Documents for a Poland visa application
The documents typically required for Poland's national (D-type) long-stay visa and for an in-country residence permit application.
Important
Poland's national visa (the D-type visa) is the document that lets a third-country national enter and stay in Poland for a total of more than 90 days for a specific long-stay purpose such as work, study, or family. It is issued by a Polish consul abroad, and the exact list of supporting documents depends on the purpose of stay and on the consulate handling your case, so you should always confirm the checklist with the specific Polish consulate.
If you are already lawfully in Poland and want to stay longer, the equivalent route is a residence permit (most commonly a temporary residence permit), which is applied for in-country at the voivodeship office. Both routes share a common core: proof of who you are, why you are staying, that you can support yourself, and that you have health cover.
Core documents for a national (D) visa
Requirements vary by consulate and purpose, but the common core the Ministry of Foreign Affairs and consulates list includes:
- A completed and signed national visa application form
- A valid travel document (passport) issued within the last 10 years, valid for at least 3 months beyond the intended stay, with at least two blank pages
- A recent biometric photograph meeting the specification
- Documents confirming the purpose of stay (e.g. an employer's declaration or work permit, a university admission letter, or family documents)
- Travel medical insurance valid in the Schengen area with minimum coverage of EUR 30,000, or Polish public health insurance
- Evidence of sufficient financial means to cover living costs and return travel
- Documentation on accommodation for the planned stay
Documents for an in-country residence permit
A temporary residence permit application is submitted to the voivode (provincial governor) and typically requires:
- A completed application form for a temporary residence permit
- A valid travel document (and, in most cases, copies of all its used pages)
- Biometric photographs to the required specification
- Documents evidencing the specific basis for the stay (e.g. employment, study, business, or family ties)
- Proof of health insurance and of a stable, regular source of income
- Proof of a place of residence in Poland
