Documents you may need
- Consultation / consent / appointment letter from an enlisted Pakistani hospital, naming any accompanying medical attendant.
- Passport valid for at least six months, plus a compliant photograph.
- For foreign nationals of Pakistani origin: a copy of the NADRA card with a notarised English translation.
- Supporting evidence of the medical purpose of travel as requested on the portal.

No. Treatment must be at a hospital that is enlisted on the Pakistan Online Visa System, and a consultation and consent letter from that enlisted hospital is mandatory.
